Online Enrollment

Thank you for your interest in PD GENEration: Mapping the Future of Parkinson’s Disease.

Below are the steps you will follow if you choose to participate in PD GENEration:

Consent at Your Own Pace – Watch our online consent video and complete the study consent form when you are ready.

Complete the Online Survey – Fill out a brief survey at your convenience. A study member will follow up to confirm your enrollment.

Collect Your Blood Sample – Use the at-home blood collection kit with guidance from a step-by-step video and live support if needed.

Send Your Sample – Schedule a UPS pickup online or by phone using the instructions in your kit or drop it off at your nearest UPS location.

Review Your Results – Meet with a genetic counselor online to go over your results and what they mean for you.
