To enroll online in PD GENEration, review the information and steps below to choose how to enroll. Select “enroll myself” below to complete enrollment online, or enroll with assistance by scheduling a virtual enrollment visit with one of our sites.

Both paths lead to PD GENEration enrollment, but now you have the option to decide how you want to enroll:

Option 1: Online Enrollment with Assistance

Step 1: Consent Meet with a study team member who will guide you through the study consent. Review and complete the study consent at your own pace using our online consent video.
Step 2: Survey A study team member will help guide you through the online survey. Complete the online survey at your own pace. A study team member will contact you to confirm your enrollment.
Step 3: At-Home Blood Collection A study team member will virtually help you complete the at-home blood collection kit. Complete the at-home blood collection kit by yourself, with assistance from a guided video and our live support service.
Step 4: Shipping Your Blood Sample A study team member will help you schedule a UPS pick-up or help locate your nearest UPS location for package drop-off. Schedule a UPS pick-up online or by phone using the instructional postcard in your kit or locate your nearest UPS to drop off your package.
Step 5: Genetic Counseling Meet with a genetic counselor online to review your results. Meet with a genetic counselor online to review your results.
